The procedure

How a beard transplant works

The Follicular Unit Extraction (FUE) method used for scalp hair transplants is also used for beard transplants. It is a minimally invasive technique used to re-establish hair thickness across various regions of the face. It can take longer than scalp work because placement of the grafts is more difficult, and the time varies depending on the size of the area treated and the number of follicles required. A goatee may only need up to around 1,500 follicles, while a full facial hair transplant may need considerably more.

During surgery the surgeon works from the donor area along the sides and back of the head, as this is the permanent zone protected from DHT. That is the area follicles are taken from for facial hair, moustache or sideburns. A FUE beard transplant is a one day surgery and allows minimal downtime afterwards. Once the ideal donor area is identified, every hair follicle is taken out in turn. We use smaller punches to trim around the follicle because we need smaller grafts for implanting facial hair. This part of the procedure is done discreetly and will be unnoticeable once the donor area heals.

When that is finished, local anaesthetic is administered to the areas of the face where the beard transplant will be carried out. Caution and skill are needed to guarantee the extracted hairs are then embedded, one follicle at a time, in exactly the right place. Done properly this results in excellent thickness and a natural looking beard.

    Beard transplant questions

    Will the transplanted hair grow like the rest of my beard?

    Yes. The hair comes from your scalp, so it grows continuously at first and needs trimming, but it takes on beard characteristics over the following months and blends in with your existing growth.

    Is a beard transplant permanent?

    Yes. The follicles are taken from the DHT resistant donor area, so once they are established they behave like the rest of your permanent hair.

    Can it cover scars?

    Yes, and it is one of the most common reasons people come to us. Grafts can be placed into scar tissue, though density may be planned differently depending on how the scar has healed.

    What is the downtime?

    It is a one day procedure and most people are back to normal activity quickly. There is some redness and small scabs in the first week, and your coordinator gives you a written aftercare timetable before you leave.

    Do you treat transgender patients?

    Yes. We carry out beard transplants as part of gender affirming treatment, and our surgical team has experience with these cases.

    Is it more painful than a hair transplant?

    Slightly. The anaesthetic goes into your lower cheek and jaw rather than your scalp, and the skin there is thinner, so the injections can be uncomfortable closer to the mouth. Once the anaesthetic takes effect you will not feel it. The average pain score for a scalp transplant is 3, whereas for a beard transplant it is around 6 out of 10.

    Is the aftercare different?

    Yes, though if anything it is easier than for a scalp transplant. Your patient coordinator is on hand at every step and you get a personalised timetable telling you what to do after surgery. The main thing to note is that we recommend eating softer foods for the first 7 days, as chewing hard foods can dislodge follicles.

    Do I have to have FUT?

    For beard transplants you can have either FUT or FUE, and it is a personal preference, though we generally prefer to recommend FUE for beard work. Your consultant will recommend what will give you the best result at your consultation.

    Does the hair grow like beard hair if it is taken from the head?

    Not to start with. The hairs need trimming at first and it takes a few months for them to gain memory and stop growing like scalp hair. The incisions are made in the direction beard hair should grow, so the growth angle is natural straight away. It is the hair type and length that take a few months to match the rest of your beard.

    How long does it take to heal?

    Beard transplants heal remarkably quickly. Scabbing comes away in around 4 to 5 days and by 7 days it looks close to normal. We still recommend two weeks off work for the redness to fade completely.
